Preparing for Botox Injections for Migraines

Before your Botox treatment, it is essential to inform your healthcare provider about all medications and supplements you are currently taking, as some may increase the risk of bruising at the injection site. Additionally, please avoid consuming alcohol for 24 hours before your appointment, as this can heighten the risk of redness and bruising.

For optimal results and a smooth procedure, please wear loose, comfortable clothing that allows easy access to your forehead, neck, and shoulders, as these are common areas for injections. Avoid makeup around the injection sites, tight headbands or hats that could pressure the treated areas, and heavy perfumes. If injections include the neck or shoulders, opt for short-sleeved or sleeveless tops, and remove any jewelry around the neck and ears to make the process smoother and more efficient.

What to Expect During the Procedure

Botox treatment for migraines involves administering small amounts of Botox through a fine needle into specific areas of the head and neck. Depending on your individual needs, multiple injection sites may be required.

This is a minimally invasive, outpatient procedure, allowing you to return home the same day. While some patients may experience mild discomfort or a slight stinging sensation during the injection process, it is generally well tolerated and brief. A topical numbing agent may be applied to minimize any discomfort. Anesthesia is not typically required unless the treatment involves specific conditions beyond migraines.



Post-Treatment Aftercare

Following the procedure, most patients are able to resume normal activities unless otherwise directed by their healthcare provider. To minimize potential side effects such as swelling, redness, or bruising, we recommend the following precautions:

  • Avoid rubbing or applying pressure to the treatment area for 12 hours.

  • Remain upright for 3 to 4 hours after the procedure.

  • Refrain from strenuous physical activity for at least 24 hours.


Additionally, for the first 24 hours post-treatment, please avoid:

  • Vigorous exercise, such as jogging or gym workouts.

  • Washing your hair (consider doing so the day before your procedure).

  • Cleansing the treated areas of skin.

  • Exposure to extreme heat, such as hot baths, spas, or saunas.


Some patients may experience tenderness or mild bruising at the injection sites. It is also possible that your migraine symptoms may temporarily worsen or that you may experience a migraine episode shortly after the treatment. You may continue to use your usual acute migraine treatments, such as painkillers or triptans, during this period.


The therapeutic effects of Botox typically begin to develop within one to two weeks. However, as Botox treatment for migraines consists of two sessions spaced three months apart, the full benefit may not be experienced until after the second session. A lack of immediate improvement following the first session does not indicate the treatment is ineffective. We recommend maintaining a daily headache diary to track your symptoms, which can be reviewed at your next appointment.
